Purpose & Instructions
This evaluation form aims to assess key aspects of COTHM campuses, Please rate each section on a scale of 1-5 based on observations, and provide comments where applicable to support your ratings. Your feedback will help identify strengths and areas for improvement to maintain high standards across all campuses.
The campuses will be awarded the following appreciation levels based on their scores:
- 81%–100%: Excellent
- 61%–80% : Good
- 61%–80% : Good
- 01%–40% : Poor
